Guardians and family members can be critical advocates for preference based person-centered care. This section of the website introduces guardians and advocates to the concept of person-centered care and then describes a three-step process to advocate for improved quality of care for older adults receiving long-term services and supports.
Our advocacy toolkit and webinar provide guidance on:
- Assessing an individual’s preferences with the Preferences for Everyday Living Inventory (PELI)
- Participating in the care planning process
- Partnering with care teams to enhance person-centered care
- Learning about preferences when a person cannot communicate through words
- Handling preferences that involve risk.
These resources are available free of charge.
