Me & My Wishes aims to improve end-of-life care by sharing videos of residents living with dementia as they discuss their preferences. The study examined the alignment between residents’ end-of-life treatment and psychosocial preferences with family and staff knowledge of those preferences. Thirty-six assisted living and nursing home resident videos were shared with family and staff during care plan meetings. Results show improved concordance at the time of video sharing, indicating the efficacy of the intervention. However, concordance diminished at follow-up, suggesting the need for ongoing conversations. Barriers include assumptions about caregiver knowledge and organizational constraints. The study underscores the importance of routine preference assessment and documentation to ensure person-centered care.
